Description

Lyn Weinberg, MD is a board-certified Geriatrician and proudly serves as the
Division Director for Geriatrics at Allegheny Health Network (AHN) in
Pittsburgh, Pennsylvania. She currently serves as the Medical Director of
AHN’s AGS Co-Care Hospital Elder Life Program (HELP) and the AHN Aging
Brain Care Program, and has been a leader in the implementation of Age-
Friendly care practices at AHN.

Objectives:
1. Define delirium, describe the intersection of delirium and dementia, and
highlight the impact of delirium on patient outcomes and healthcare costs

2. Introduce the Hospital Elder Life Program (HELP), a multicomponent
intervention to prevent delirium

3. Discuss implementation of HELP and other delirium
prevention/management strategies to promote Age-Friendly care

4. Discuss challenges
