2022-23 Specialized Geriatric Services Grant Recipient

The RGPc is pleased to announce the recipient of the RGPc Specialized Geriatric Services Grant is Dr. Justin Lee for his project, "Development and Implementation of Delirium and Dementia-Friendly Mobile Comfort Carts to Increase Senior-Friendly Non-Pharmacologic Care: A Mixed Methods Pilot Study."  Congratulations Dr. Lee and team! We look forward to seeing the impact of this important work to support older adults receiving care at St. Joseph's Healthcare Hamilton.

Geriatric Foundations is a new e-Learning program by the Regional Geriatric Program Central designed to provide healthcare professionals with a foundation to geriatric care. 

Care for Older Adults: Where to Start

The Regional Geriatric Program Central Patient and Caregiver Education Working Group have developed the Care for Older Adults: Where to Start tool. This tool provides a selection of helpful provincial and regional resources for older adults and their caregivers who are seeking support. Click here to download the tool.

Behavioural Supports My Transitional Care Plan (MTCP) Tool 

My Transitional Care Plan (MTCP) summarizes meaningful information that contributes to the facilitation of successful transitions across sectors. It was designed for use with older adults presenting with, or at risk of, responsive behaviours/ personal expressions associated with dementia, complex mental health, substance use and/ or other neurological conditions. MTCP can be used to share information between members of  person's care team from both their existing and new location. Any healthcare provider who has assessment and/ or care planning in their scope of practice can fill it out. 

MTCP was adapted from My Transitional Care Plan during the COVID-19 Pandemic (BSIT Collaborative, 2021) and My Transitional Care Plan (North Bay Regional Health Centre, 2019). 

The tool is available for download as both a Microsoft Word and PDF Fillable Form. Supporting resources are also available, including guidelines for use and completed examples. English & Français

PGLO Caregiving Strategies Handbook

The Caregiving Strategies Handbook: Providing Care and Support for a Senior Living with Frailty was developed for caregivers, and the family and friends who support seniors experiencing frailty. Created with advice from caregivers across Ontario, this handbook has been reviewed by health care professionals.

RGP Central and Niagara Health, in affiliation with the Division of Geriatric Medicine at McMaster University are inviting applications for a full-time community-based geriatrician.
